G2 Form ( OA to and from)

 PLEASE UNDERSTAND WHEN YOU REQUEST A SAFETY RESOLUTION FORM, YOUR SUPERVISOR IS GOING TO PROVIDE THE DOCUMENT. 

•If your Supervisor does not give you a form, one can always put it over the radio to Bus Command Center or Rail Control Center (R.C.C.).

Supervision is always suppose to have this form readily available, ALWAYS. 

•After completing the first step (numbered 1), the Member should follow the next step (numbered 2). Please understand when you are requesting a Safety Dispute Resolution Form, you may be met with aggression from Supervision. Do not match their energy and engage in their conduct unbecoming, call Bus Command Center or Rail Control Center and report the incident.
